
Louisiana State University offers this comprehensive Student Health Insurance program for students and their eligible dependents. The plan features medical coverage underwritten by Nationwide Life Insurance Company, pharmacy benefits by WellDyneRX, dental and vision coverage by AlwaysCare. The plan offers a variety of services provided though networks such as Verity, BestCare, Beech Street, AlwaysCare Dental and Vision. Additional benefits include Medical Evacuation, Repatriation, Medical Travel Assistance, Preventive Services, Wellness and Disease Management Nurse Hotline and the LSU Student Health Ticket*.
All students at Louisiana State University and Agricultural & Mechanical College and LSU Law Center students who are full and part-time registered for resident study (and taking classes at LSU and LSU Law Center), excluding individuals registered for or in any of the various courses and activities offered as “Continuing Education” programs of the University, are eligible to purchase the plan for themselves and their dependents. Eligible dependents for the purpose of this insurance are defined as the legal spouse and any unmarried children, including grandchildren in the legal custody of the grandparent, under 21 years of age (under 24 years for unmarried full-time students who remain wholly dependent upon the participating student). Dependents’ eligibility expires concurrently with that of the insured student.
Students can enroll into the student plan thru PAWS by 9/12/08 (PAWS Enrollment Codes: 05 001 Basic Fall Semester; 05 002 Standard Fall Semester; 05 003 Enhanced Fall Semester; 05 004 Basic Annual; 05 005 Standard Annual; 05 006 Enhanced Annual; 10 001 Vision Fall Semester; 10 002 Vision Annual; 11 001 Dental Fall Semester; 11 002 Annual). Students may add dependents to their policy, review claims information, and enroll after PAWS deadline at LSUstudentinsurance.com. Coverage becomes effective the date the student enrolls on www.LSUstudentinsurance.com.
Graduate Assistants who hold a full-time (50% effort) assistantship for any semester will be eligible for a Medical Coverage subsidy of $350 for each semester paid by LSU. (Graduate Assistants must enroll via PAWS to receive their subsidy. The subsidy is credited to the assistant’s account after enrollment into the program.) * The LSU Student Health Ticket is an online resource that manages student referals, locates in-network providers, outlines program benefits, and creates provider specific ID Cards. Students who have not received an ID Card can print a Temporary ID Card from the LSU Student Health Ticket.
